Making an Appointment

To schedule a consultation with one of the oral surgeons, referred or new patients may call our clinic coordinator at (612) 624-4435 and select option 1 when prompted. Patients should have insurance and referral information at hand when they call.

After the initial appointment is scheduled, patients will receive an information packet through the mail that will include directions to the clinic and registration materials that they may fill out in the comfort of their own home. Or if you prefer, you may download the forms as pdf files here.

At the first appointment, patients should bring any relevant records from other providers, including radiographs (x-rays), CTs, MRIs, copies of medical and dental records, and any communication from the referring doctor. Bringing these materials will help the oral surgeon to get as full a picture as possible and enable the initial consultation to be as beneficial as possible.

For Your First Appointment

• Please arrive 30 minutes early for your first appointment to register
• Bring relevant records from other providers such as radiographs (x-rays), CTs, MRIs
• Bring copies of medical and dental records
• Bring any communication from the referring doctor
• Bring your Insurance card and Photo ID to every visit as either a new or as an established patient
