How to Make Payment

We are participating providers for most major medical and dental plans in Minnesota. Patients or referrers are requested to contact our business office at (651) 842-3363 or to contact the individual insurance company with questions about participation in a particular plan. Please also call with any questions about the need for a referral from the patient’s primary clinic.

Co-pays, if required, are due at the time of service. Prior to the patient’s first appointment, our business office will verify the patient’s insurance coverage and contact the patient if other payment is required.

After a treatment plan is developed with the oral surgeon, our staff will work with the patient to determine whether his or her insurance company requires pre-authorization or pre-determination for the planned procedure.
